How to Sign Up for a Biometric Screening

Log in to your Live Well W&L account, click on MY HEALTH in the upper right-hand corner (or look for the heart icon if using the mobile app), Biometrics Enroll Now, and then Enroll in Biometrics. (Note: Make sure to enter your phone number on the Quest “My Account” page.)

There are three options:

1. On-Campus: Under At an Event click on “Make an Appointment.” For the location, enter 24450. View detailed instructions.
2. Local Quest Lab: Screenings at a Quest lab (closest one is 204 E. Washington Street) can be scheduled under At a Patient Service Center. The dates/times are very flexible. View detailed instructions.
3. Through your PCP: Under Physician Results Form click on “Order Form.” In this option, you’ll download and print the form, and take it with you to your doctor, who will request the bloodwork. Then to get your Essential token, you simply enter the date of the bloodwork in “Know Your Numbers” under the ANNUAL PHYSICAL option. You have the choice of uploading the form or not – you get credit for it either way, as long as you enter the date of your bloodwork in the ANNUAL PHYSICAL category.

Note: Note: For options 1 and 2, it’s FREE to you. Quest will automatically upload your results to your Live Well W&L account (W&L cannot access your results – they’re confidential) and Sonic Boom will automatically log your Essential biometric screening token.

Helpful Tips

• Do not use Firefox as your browser.
• Make sure to enter your phone number where requested.
• Need to RESCHEDULE or CANCEL? Log in to your Live Well W&L account, click on MY HEALTH in the upper right-hand corner (or the heart icon), BIOMETRICS ENROLL NOW, and then ENROLL IN BIOMETRICS. You will be transferred to the Quest Diagnostics webpage. Click on RESCHEDULE or CANCEL and follow the instructions.
