If you are a SINGLE parent with a child on your W&L health insurance policy, you may be eligible for the highest Live Well W&L discount rate: $75 per month.
To qualify, complete the Single-Parent Discount form and meet all of the following criteria:
- You have completed the necessary wellness tokens to earn the Live Well W&L discount for the upcoming year, or you are currently receiving the discount;
- You are single, legally divorced, or widowed;
- You do not have a Declaration of Domestic Partnership on file with W&L;
- You have one or more children enrolled on your W&L health insurance plan.
Notify the Office of Human Resources within thirty (30) days of a change in marital or parental status.
This discount may be renewed annually.
