Personnel Action Form

Complete the Personnel Action Form to notify Human Resources of any personnel changes. You may use this form to inform us of hiring, transfers, promotions, departures, reclassifications, title changes, budgeting changes, supplemental compensation, hierarchy changes or leaves.
