The Camden Coalition of Healthcare Providers (CCHP) seeks to fundamentally change how providers, office staff and community agencies in Camden care for city residents with diabetes by building an accessible, high-quality, coordinated and data-driven health care delivery system with a strong primary care base. While the current system is largely fragmented and broken, for the past six years, CCHP has made major gains through citywide relationship-building, cross-site data-sharing, and the implementation of key Health Information Technologies (HIT).
