RE: SUPPLEMENTAL DISABILITY INSURANCE DEADLINE

I would like to remind all employees that today, March 30, is the deadline for enrolling in the new Voluntary Supplemental Disability Income Insurance program without medical underwriting. The Office of Human Resources must receive your entire enrollment packet with all sections filled out and signed if you wish to participate.
If you are unable to bring the original copy to the Human Resources office today, then please fax the form to x8060 and bring the original copy in on Monday. However, if we receive neither the original or fax, you will not be able to enroll without medical underwriting in the future. Finally, the HR office does not have additional copies of the individual packets sent out earlier this month.
