Required Forms

For Appalachian Adventure Participants

The Appalachian Adventure Leading Edge Application will be available on GHQ June 3, 2014 at 11 a.m. EDT through June 11, 2014 at 4 pm EDT.  The required forms below are specific to the Appalachian Adventure trips.  If assigned to an AA trip after applying to The Leading Edge program, you will receive an e-mail alert at the end of June notifying you of your trip assignment, with instructions on completing and submitting these trip-specific forms.

All forms must be completed by the accepted Leading Edge participant and returned to the Office of the Dean for First-Year Experience by the end of July. Send to:
