This is a summary of the annual report of the Washington and Lee University Health and Welfare Plan, EIN 54-0505977, Plan No. 510, a welfare benefit plan, for period July 01, 2012 through June 30, 2013. The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

Washington and Lee University has committed itself to pay certain health care claims incurred under the terms of the plan.

Insurance Information

The plan has contracts with Anthem Health Plans of Virginia, Inc., Humana Insurance Company, and Life Insurance Company of North America to pay health (includes a PPO), vision, life insurance, long-term disability, and prescription drug claims incurred under the terms of the plan. The total premiums paid for the plan year ending June 30, 2013 were $8,660,919.
